INSURANCE
We accept numerous insurances plans including those offered by networks such as:
- DIRECT
Alignment, Medicare, United Health, Humana, Multiplan (& PHCS), First Health
- VIA NETWORKS:
Choice Care (via Humana), CIGNA (via PHCS), Kaiser (via Multiplan/PHCS),
Merchant Benefit (via First Health), Oxford (via United), PHCS Network,
and more.
SELF PAY
Some patients find themselves temporarily without coverage (or) have coverage in a health plan that we are not in network with and yet they are unable find doctors in their network who can provide the immediate and attentive care that they need and we offer. In such cases, patients can self-pay with cash or credit card.
Patients who have coverage in a health plan that we are not in network with and choose to self pay for their visit with us will generally be able to expect that their lab orders, imaging, specialist referrals and prescriptions will still be paid by their primary insurance. Only the visit fee to our office will not covered by non-contracted insurance.
For a low visit fee, you get same-week, often same day, access to experienced caring Primary Care Physicians. Why wait months to take care of your health, risking unfavorable complications or illness? You know you are worth it!
Prompt Pay Discount Rates
Thank you for allowing us to care for you and your family.
Our Prompt Pay Discounted Rates are as follows:
- $350 -New Patient First Visit (1 Full Hour Visit with Physician)
- $285 -Existing Patient Return Visit (Visit Length Varies according to need)
These rates are only available to patients who pay in full for their visit at the time of care by either cash, or credit card.
